Cardiopulmonary resuscitation (CPR) given by a bystander before emergency services arrive can more than double the person’s chance of survival. CPR isn’t just a medical skill, it is a community skill, one that can save loved ones, neighbours, co-workers and strangers.

However, research has revealed a striking and concerning pattern: women are less likely than men to receive bystander CPR after a cardiac arrest. On International Women’s Day, it is important not only to celebrate women’s contributions but also to ensure equity in life-saving care. Understanding why this disparity exists and how we can address it is essential for building safer communities for everyone.

What the statistics show: a real gender gap exists

Several large studies from outside South Africa (in the UK and the United States) highlight a consistent trend: women experiencing out-of-hospital cardiac arrest are less likely to receive CPR from bystanders compared with men. For example:

Researchers analysing more than 300,000 cardiac arrest cases in the U.S. found that women were about 14 % less likely than men to receive bystander CPR or defibrillation.

Earlier research also showed women in public locations were less likely than men to receive CPR, a difference not fully explained by the setting or other factors.

These differences have real consequences: when CPR isn’t given promptly, survival rates drop sharply. That means some women who could survive with early intervention may not get that chance.

This fear, rooted in social norms about physical contact, can delay action that should be immediate.

2. Misconceptions about symptoms and conditions

Women sometimes display different symptoms during cardiac arrest or a heart attack, making it harder for bystanders to recognise the emergency and act quickly. While this has been more studied in heart attacks, similar confusion can delay CPR recognition and response.

3. Training biases

Traditional CPR training videos and mannequins tend to depict male torsos, which may subconsciously influence people’s confidence or comfort when performing CPR on women. This has led to initiatives like the “Womanikin,” which adapts training models to better represent female anatomy and help learners practise on all body types.

Sudden cardiac arrest doesn’t discriminate by gender, and South Africans are just as likely to witness a loved one collapse in a public space, in a shopping centre, at school, or at home.

What does differ is people’s confidence and willingness to act. In many communities, hesitation often comes from fear: fear of doing it wrong, fear of touching someone’s body, or fear of legal consequences. These fears can be heightened when the person in need is a woman. This means we may be inadvertently reducing survival chances for women simply because people aren’t confident in responding.

Communities that openly discuss and practise CPR are more likely to help when it matters most.

Making training widely available

Whether in schools, workplaces, community centres, or via public education campaigns, everyday people need opportunities to learn how to respond confidently to cardiac arrest. Many South Africans have lived through that anxious waiting period: a loved one struggling to breathe, a child injured at sport, an elderly parent collapsing at home, or a car accident on a busy road. In those moments, panic is natural. But so is the realisation that help may not arrive as quickly as we hope.

South Africa’s Emergency Medical Services (EMS) system is under strain, and the impact is felt in everyday homes, schools, workplaces, and communities. This is exactly why basic first aid knowledge is not “nice to have” anymore. It is a practical life skill that can buy time, reduce harm, and in some cases, keep someone alive until professional help arrives.

Since 1935, the South African First Aid League has focused on equipping ordinary people with calm, practical skills for extraordinary moments. This article explains what we know (with verifiable data) about the EMS pressure, and what first aid skills can do in the gap between an emergency and an ambulance.

In other words, the system is attempting to serve millions of people with fewer vehicles than required to meet demand.

In reporting that includes comments attributed to a Department of Health spokesperson, it is stated that only 4,007 ambulances were operating across all nine provinces, against an estimated need of more than 6,000 based on population and a recommended coverage ratio.

When ambulances are limited, callouts queue up, rural distances become even harder to cover, and response times can stretch. The painful truth is that in some emergencies, the first responder may be a parent, teacher, colleague, neighbour, or passer-by.

A government technical report notes targets of reaching Priority 1 patients within 15 minutes in urban areas and within 40 minutes in rural areas.

Those targets matter because they reflect what emergency care aims to achieve. For some, they show up as a mild skin rash or hay fever. For others, an allergic reaction can escalate quickly and become frightening, even life-threatening. What makes allergies particularly challenging is that they do not always follow a predictable script. A reaction can happen at home, at school, at work, or while enjoying a meal with friends.

Understanding first aid in relation to allergies is not about fear. It is about awareness, preparation, and knowing what to do in those critical moments when time matters.

Common allergens include foods such as nuts, shellfish, dairy, and eggs, insect stings, medication, latex, and environmental triggers like pollen or dust.

Reactions can range from mild symptoms such as itching, swelling, sneezing, or stomach discomfort to severe reactions known as anaphylaxis. Without prompt treatment, it can be fatal.

What makes allergies particularly dangerous is that a person may not realise how severe their reaction could be until it happens.

This year’s theme, “Diabetes and Well-being”, highlights how awareness, compassion, and basic first aid knowledge can empower all of us to support the health and safety of those living with diabetes.

Across South Africa, thousands of people manage their blood sugar daily with dedication and discipline. Yet even the most careful individuals can face sudden medical emergencies, and in those moments, what happens in the first few minutes can make all the difference.

Understanding Diabetes and Its Everyday Challenges

For many people, this means a lifelong balance between food intake, exercise, and medication.

However, daily management isn’t always predictable. Stress, missed meals, physical activity, or changes in medication can cause blood sugar levels to rise or fall rapidly, leading to emergencies that require swift, informed action.

The two most common diabetic emergencies are:

Hypoglycaemia (Low Blood Sugar): When blood glucose drops too low, often caused by too much insulin, missed meals, or intense exercise. Symptoms may include trembling, sweating, confusion, dizziness, irritability, blurred vision, or sudden fatigue.

Hyperglycaemia (High Blood Sugar): When glucose levels rise too high, usually due to missed medication, dehydration, or infection. Symptoms can develop gradually: excessive thirst, frequent urination, drowsiness, and, in severe cases, unconsciousness.

For someone unfamiliar with diabetes, these symptoms can easily be mistaken for fatigue or stress. That’s why first aiders play such a vital role. Recognising the signs early can prevent a crisis from becoming life-threatening.
